Incident Report Form
Incident Reports are not for complaints against refs, coaches, board members, etc. This form is for concerns, injuries, or any incident that is outside the scope of a complaint against someone directly.

24-Hour Rule
We kindly ask that all non-urgent concerns or incidents be reported after a minimum of 24 hours. This allows time for situations to de-escalate and for perspectives to be considered more fully. Matters involving serious threats or urgent concerns may be reported immediately and will be addressed accordingly.
Follow-Up
While all concerns and complaints are taken seriously and formally documented, not all submissions will receive direct follow-up from a Board member. Our priority is to review and address matters thoughtfully and, where appropriate, as discreetly as possible.
Confidentiality
We are committed to maintaining confidentiality in all matters submitted. Information provided will not be publicly shared with members, coaches, or players. When a concern requires further action, every effort will be made to protect the identity of the individual submitting the complaint. In certain circumstances, it may be necessary to disclose names as part of the resolution process; if so, you will be informed in advance.
